A: Download, complete and submit an Application in English (PDF) or an Application in Spanish (PDF).

Q: What benefits will my child receive in the program?

Q: How long can my child stay in the program?

A: Each eligible child may be enrolled in our dental care services program for one year. A second year of service is available, if medically necessary.

Q: What is the age range you cover?

A: Children ages one to 17 are eligible.
